Direct billing is available for your convenience with most major insurance companies. Rules
apply. Please be aware of the terms of your benefit plans and whether or not a doctor’s note is
required (some plans may require, many do not).
Debit, visa, mastercard, cash or etransfer accepted for any amounts not covered by insurance.
Receipts issued when you need to submit for reimbursement on your own.
I will secondary bill for you when possible but not all insurance companies allow secondary
billing and you may need to submit to the second plan yourself. These are not my rules, they
are the rules of the insurance company.
